Lesson 4a – Discussion

Formulate a PICOT Question

The objective of this discussion is to introduce students to Evidence-Based Practice by teaching them to pose specific questions of importance for their practice. Therefore, for this discussion, you will be developing and writing your own clinical-oriented (PICO(T)) questions as discussed in the learning module.

Instructions:

1.              Instructions:

1.     Questions are the driving force behind EBP.  If there were no questions, EBP would be unnecessary. Using the identified clinical problem in discussion #3.

2.     Formulate a PICOT question by identifying:

A.    P – Patient or Problem: Who is the patient? What are the most important characteristics of the patient? What is the primary problem, disease, or co-existing condition?

B.    I – Intervention: What is the main intervention being considered?

C.    C – Comparison: What is the main comparison intervention? (optional)

D.    O – Outcome: What are the anticipated measures, improvements, or effects?

b.     Did the research produce any practical applications?

c.     What are the nursing implications of this research?

d.     How do you evaluate the significance of the research?

13. Papers will be graded by rubric. Please take time to review the rubric so that you are aware of the expectations for the research critique paper.

DiscussionLesson 4b – Discussion

Conducting the Evidence Search

The purpose of this discussion is to teach students to use electronic databases and methodological filters to select the most relevant evidence to answer the clinical questions. A secondary purpose of this assignment is to teach students to format citations according to the American Psychological Association Style Handbook (7th Edition).

Instructions:

1.     For this discussion, you will use the PICO question you generated for the previous discussion (Use the PICO worksheet attached to this discussion).

2.     Select the most appropriate databases using information from the MDC Library databases.  Explain in a short paragraph why you selected each one.

3.     Find synonyms for Population, Problem, Intervention, Comparison, Outcome. Fill out the list with synonyms.

4.     Using the advanced search feature, conduct separate searches for each of the above PICO, using “OR” with synonyms. Check for the number of hits for each search and revise as necessary.

5.     Using the search history feature, combine PICO searches, using “And.”

                                                I.          Make note of the number of hits.

II.          Revise the searches combined, if necessary.

6.      Use methodological filter terms, first for systematic reviews, randomized control trials, cohort studies, etc. Note the number of studies found after applying the exclusion and inclusion criteria).

7.     Limit your searches to five (5) years.

8.     Write a paragraph describing the process ( this is the method section of the literature review).

9.     Title the discussion with the PICOT question.

10. Initial post: Your post should be within a range of 200-300 words.

11. Peer response: Identify two (2) students that may share a similar question and make significant comments in their Search methodology (can it be replicated?).

12. Be active because it may help you form a group for the EBP paper.

13. Cite your references APA 7th  ed. (not included in the length of the post).

14. To see rubric, click on the 3-dot menu  on the top-right side of screen.

DiscussionLesson 5 – Discussion

Locate and Identify the “Best Levels of Evidence”

The final step to the perfect PICO search is to determine the level of evidence within each relevant article. In searching for the best available evidence, a hierarchy exists regarding the level and strength of evidence (see Fig. below). As you review the journal articles found in discussion 4, select those that are based on the highest level of evidence.

2.     Title the discussion with the PICOT question.

3.     Initial post: Your post should be within a range of 150-200 words and respond to the following question?

1.     Identify the level type of category of the evidence for the literature review.

2.     Now that you see all the studies together identify the overall strength of the evidence (strong, compelling, etc.).

3.     Now ask yourself what can be implemented why or why not (feasibility).

4.     Peer response: Identify two (2) students that may share a similar question and share with them the purpose of your literature review (1 sentence eg. The purpose of this review is to examine the effects of hydrotherapy on reducing foot edema among chronic CHF diagnosed patients). Comment on their purposes and invite them to join your EBP group.

5.     Cite your references APA 7th  ed. (not included in the length of the post).

Module 1 – Discussion

Health Care Innovation Changes

In 2019, community health workers relied heavily on in-person interactions, home visits, and paper-based or basic electronic documentation to coordinate patient care. Much of their work involved physically traveling to clients’ homes, conducting health screenings, providing education, and connecting individuals to resources. Although effective, these methods were time-consuming and limited by geography, transportation challenges, and scheduling constraints. Follow-up could also be inconsistent if patients missed visits or lived in remote areas.

Today, advances in mobile health (mHealth) applications and telehealth technology have transformed how similar tasks can be accomplished. Many assessments that once required in-person visits can now be completed through secure video calls, app-based surveys, remote monitoring devices, and automated reminders. Community members can receive health education instantly through phone apps, while digital platforms allow seamless communication between patients, nurses, and interdisciplinary teams. Apps can track blood pressure, glucose levels, medication adherence, mental health symptoms, and lifestyle behaviors in real time, allowing healthcare providers to intervene earlier when concerns arise.

This shift increases accessibility, especially for individuals with mobility limitations or those living in rural or underserved communities. Telehealth also enhances continuity of care by enabling consistent follow-up, reducing missed appointments, and empowering patients to engage actively in their own health management. While community health workers remain essential, technology now amplifies their reach and efficiency, ultimately improving population health outcomes.

Discussion:

How do you believe the shift from traditional community health work in 2019 to the use of mobile health apps and telehealth has improved patient care and access? In your response, discuss at least three ways this new trend is better.

1.     Write an original post (at least 240 words or more) on your thoughts, feelings, and examples. Use at least one reference to support your ideas.
